Finding out that you are pregnant can be an overwhelming, emotional and an exciting time. 

For some, the excitement takes time to settle in while they face the initial sense of overwhelm.

It’s ok, you’re not alone. 

We’re here to walk you through the first trimester, step by step and will cover everything you need to know from the first steps after that positive pregnancy test to what to expect in your first trimester screening.

How Long is the First Trimester?

The first trimester is 12 weeks, calculated from the first day of your last menstrual cycle.

By the time you have a missed period, you are already 4 weeks pregnant. Many woman do not discover they are pregnant until 5-6weeks. 

What are the Early Signs of Pregnancy?

There are many signs of pregnancy, even from very early on. However, many of the symptoms may feel the same as an approaching period.

We asked our community of mothers what pregnancy symptoms they experienced in the first trimester.  Here’s what they said:

  • Missed period
  • Tender Breasts
  • Nausea or vomiting (see our guide to easing morning sickness) 
  •  Fatigue
  • Bloating
  •  Cramping
  • Increased Urination

When to Take a Pregnancy Test?

If you have been trying for a baby or think you may be pregnant, you know that the wait between ovulation and a period can feel like months.

You can take a pregnancy test from the first day of your missed period. Some tests allow you to test earlier (believe me, I was testing), however many say the most accurate result is 3 days after a missed period. 

Prenatal Vitamins, Substances and Food in Pregnancy

If you have received a positive pregnancy test, it’s time to consider a few health changes.

Firstly, consider what prenatal vitamin to take (if you are already not on one), research foods to avoid in pregnancy and foods that support optimal health for both you and your baby.

You should also begin to limit your caffeine intake and refrain from substances such as alcohol, drugs and cigarettes. If you are unsure, consult with your healthcare proffesional.

Congratulations! By now, you have already had a positive pregnancy test. So what next? the first steps after a positive test include: confirming your pregnancy with a blood test at your Dr, registering your pregnancy, booking your first antenatal appointment, choosing your mode of care and beginning your first trimester screening. In just a few weeks, you’ll be hearing your babies heart beat for the first time, see them for the wriggle around on your initial ultrasounds and possibly even find out the sex of your baby. We’re so excited for you on this journey. Every pregnancy is different, but here is what you could expect in your first trimester screening. 

Initial Blood works in pregnancy

Your initial blood works is an important part of the first trimester screening. Your Dr will request complete blood works to check your glucose levels, identify your blood type and Rh factor, check your ferritin levels, check for immunity to rubella, and test for HIV and  hepatitis B.

What to expect first trimester

Dating Scan

If you are unsure of your dates, your Dr may refer you to a dating scan. This scan is done around 7-8 weeks of pregnancy. During this scan you will be able to confirm your due date, see your baby beginning to form and listen to their heartbeat for the first time.  

NIPT (Harmony Test)

You may wish to also get the NIPT (non invasive prenatal testing). This is an additional test at the cost of the parent consisting of a simple blood test to screen for genetic conditions such as down syndrome. This test occurs around 10 weeks gestation and can also include the sex of your baby. 

First Trimester Ultrasound and Screening Test

This is often referred to as the combined first trimester screening test and consists of two parts. 

– A blood test (maternal serum screening test)
– Ultrasound (Nuchal translucency)

This combined screening occurs around 11-13weeks with the blood test a few days prior to your 12 week scan. This test also provides indication if your baby is at risk of having a genetic condition such as down syndrome. 

This may be the first time you see your baby, or if it isn’t, you’ll notice how much your baby has grown already.

What to Expect at Your First Antenatal Appointment

Once you discover you are pregnant, you should have registered your pregnancy and be booked in with your zoned hospital for your first Antenatal appointment. This appointment is often conducted in the woman’s health area of your hospital with a registered midwife. At your first appointment you may expect:

  •  A mid stream urine sample.
  • Personal history conducted.
  • Screening for smoking/risks of domestic violence and mental health assesment. 
  • Discussion of suppliments and health during pregnancy. 
  • Discussion on benefits of breastfeeding.
  • Discussion on care type (in SA if you wish to join Midwife Group Practice)
  • If second pregnancy, a discussion on first pregnancy and risk of PND.
  • Blood pressure check.
